Rebound mic key does not clear the old key

Bug Description:
I originally had my in-game microphone key set as "v", which is what I use in most games.  Later, I rebound it to "middle mouse button", which also works fine.  The problem is that now when I press "v", other players still hear my voice.  In the control tab of the settings menu, it correctly shows middle mouse as the push-to-talk key, which functions as intended in addition to "v" performing the same function even though that bind is cleared.  Recently got killed several times when my voice was transmitted by a key not bound to that function, thus giving my location away.  Possible related bug: often, my voice chat reverts to "always on" without me changing it.  This has also led to getting killed because I did not realize my mic was transmitting.

Date Seen:
Several months now.  Most recently, 09/24/2017. But also several months previous to this; cannot remember the exact date, but it was before the crossbow was added.

Server: 
North America server, FPP

Troubleshooting Attempted:
I tried repeatedly clearing and rebinding the Push-to-talk key; no effect.  I had Steam verify the integrity of the game files, which passed with no issues; no effect.  I tried deleting the "GameUserSettings.ini" thus making the game generate a fresh one; no effect.  Using Ctrl+T to mute voices successfully mutes my own transmissions, however not a viable workaround because then I cannot talk to or hear other teammates/players.

Have you changed any launch / command line options for the game?  No.

System Specifications:
Operating System:  Windows 10 Pro, 64 Bit
Graphics Card: nVidia GeForce GTX 1070 (no overclock)
CPU:  Intel i5-6600k (tried with OC both on and off; same bug appears)
Ram: 16 GB
